Hunterdon-Mercer Chronic Disease Coalition

The goal is to address the state’s cancer burden and work towards improving health outcomes for people with or at risk for cancer and other chronic diseases.

  • Promotes healthy lifestyle choices, cancer screening, early detection, disease management & quality of life.
  • Educational programs are held throughout the year.
  • Cancer Surviving & Thriving Workshop
    • Developed by Stanford University, this program in self-management is proven to help cancer patients, survivors and caregivers learn to manage the challenges of thriving.
    • Addresses: fatigue, emotions, living with uncertainty, stress/anxiety, pain, etc.
    • 6 week series provided throughout the year.

Hunterdon Regional Cancer Center NJ CEED (Cancer Education & Early Detection)

Grant funded program implemented as a strategy to find cancer at an early stage before symptoms develop, prevent the occurrence of cancer and improve health outcomes.

  • Free cancer screenings are provided to:
    • Women
      • Breast: Ages 40-64, clinical breast exam and an annual mammogram
      • Cervical: Ages 21-64, a Pap test & HPV co-testing if needed
      • Colorectal: age 45 and above, a Fecal lmmunochemical Test (FIT) and colonoscopy if needed
    • Men
      • Colorectal: Age 45 and above, a Fecal lmmunochemical Test (FIT) and colonoscopy if needed
      • Prostrate: Age 50 and above, both the Prostate Specific Antigen (PSA) and Digital Rectal Exam (DRE)
    • Screenings are provided to individuals who meet or fall within the recommended ages of screening, do not have health insurance or limited health insurance, have a household income at or below 250% of the Federal Poverty Level, are a resident of NJ (regardless of citizenship status).

Jersey Assistance for Community Caregiving (JACC)

JACC Services provide:

Eligibility:

NJ residents 60 years old + who:

  • Requires a nursing home level of care, but wishes to remain home.
  • Is not financially eligible for MLTSS (Managed Long-Term Support Services) or any Medicaid programs.
  • Is not or will stop participating in the NJ Alzheimer’s Adult Day Services Program or Statewide Respite Care Program
  • Has resources that are at or below $40,000 for an individual, or $60,000 for a couple.
  • Is a US citizen or qualified permanent resident.
  • Has no alternate means to obtain the needed support.
  • Lives in the community, and not in an assisted living, nursing home or residential care facility.

Medicare Information

Medicare is federal health insurance for people 65 or older, some younger people with disabilities, and people with End-Stage Renal Disease.

  • Medicare covers 80% of your care needs. Having supplemental or secondary insurance is important.
  • Medicare Part A: Covers hospitalizations, care in a skilled nursing facility, hospice care & some home health care.
  • Medicare Part B: Covers certain outpatient doctors’ services, medical supplies & preventative care.
  • Medicare Part C: Medicare Advantage Programs
    • Examples: Aetna Medicare, United Healthcare AAPR Medicare, Medicare Blue, etc.
  • Medicare Part D: Drug Prescription Programs
    • Assistance programs: NJ PAAD (Pharmaceutical Assistance to the Aged & Disabled)
      • Typically aged 65 and older
      • Your income for 2026 is less than $54,943 if single & $62,390 if married

 

*Knowing your benefits is important. Be sure to verify with your medical providers that your insurance is in-network before you make any changes. If you do make a change, alert your medical providers immediately.

Medicaid Information

Medicaid provides health coverage to millions of Americans, including eligible low-income adults, children, pregnant women, elderly adults and people with disabilities. Medicaid Is administered by states, according to federal requirements. The program Is funded jointly by states and the federal government.

NJ Family Care

  • NJ FamilyCare is a federal and state funded health insurance program created to help qualified New Jersey residents of any age access to affordable health insurance. NJ FamilyCare is for people who do not have employer insurance.
  • Income limits for 2026 for adults
    • Family size of 1- $1,836 per month
    • Family size of 2- $2,489 per month
    • Family size of 3- $3,142 per month
    • Family size of 4- $3,795 per month

Long-term Care Medicaid

  • Helps to pay for nursing homes, assisted living, home health aide assistance at home.
  • Eligibility: $2,982 per month for single person with a resource max of $2,000
  • A 5 year look back at your finances will be required.
  • Contact your local Board of Social Services for the county where you reside to be screened.

    • Many therapists tend to accept private pay only, but may provide you with necessary documentation to submit to your insurance.

American Cancer Society: Road to Recovery Program
  • Free volunteer driver program.
    • All drivers are background checked and follow COVID compliance (vaccinated, boosted and masked).
  • Patients or caregivers can get in contact through:
  • ACS will verify if their program services your home zip code. They do service Flemington, NJ.
  • As much notice as possible is preferred. At least 48 hours notice is required.
  • Patients need to be independent or have a caregiver with them to help get in/out of the vehicle. Drivers are not permitted to assist. Medical devices such as a cane or walker are permitted, if able to be used independently.
  • Please be aware this is not a guarantee of a ride as they need to locate a driver.
Modivcare (formerly Logisticare)

  • For patients with NJ Medicaid only
  • Covered by Medicaid (Primary or secondary)
  • Modes of transport available: car (Lyft), wheelchair van, stretcher (BLS- Basic Life Support)
  • If a non-emergent stretcher is needed and you have primary insurance, you will be required to use your primary insurance instead of Medicaid.
  • How to arrange:
    • Patients and/or their families will call the ModivCare call center to request appropriate mode of transport.
    • A confirmation# will be provided. This is not a guarantee of a ride, therefore, patients should confirm prior to their appointment.
    • At minimum- 2 business days note. As much notice as possible is preferred.
Non-Emergent Medical Assisted Transport
  • Atlantic Ambulance
    • Must be arranged by a member of the Cancer Center with advanced notice.
    • At minimum 2 business days notice. More notice is preferable.
  • Private pay Wheelchair Vans vs Stretcher (BLS- Basic Life Support)
    • Wheelchair Vans
      • Wheelchair transportation provided by medical attendant
      • Insurance does not cover this form of transportation.
    • Stretcher (BLS- Basic Life Support)
      • Stretcher transportation provided by an EMT.
      • Medicare or other insurances will only cover if the patient is completely bed bound as certified by their physician.
      • Please be aware that even with medical certification, this is not a guarantee of insurance coverage.
